5 Mile Smiles is pleased to file your insurance claim on your behalf. We are contracted with the following insurances:
- Delta Dental/Washington Dental Service
- Oregon Dental Service
- Connection Dental (GEHA)
- Premera Blue Cross
- United Concordia
- United Health Care (AARP)
When you schedule your appointment, download our patient forms and provide us your insurance information, so we can start determining your benefits coverage. After your appointment, we’ll bill the insurance company for you.
Unfortunately, we are unable to accept State, Delta Care, Apple Health, Uniform 1, or Willamette.
How does billing the insurance work?
Dental insurance works a little differently than regular health insurance. Typically, insurance will pay up to a certain amount each year, and cover a percentage of restorative or endodontic care. You may have a small copay or need to pay a percentage of the cost for services like fillings, root canals, crowns, and dentures. Orthodontics may be included, but cosmetic treatments usually are not.
Call your insurance provider or your employer’s human resources department for specifics regarding what is covered in your benefits.
What if I have a balance after insurance?
In order to keep our services affordable for you, 5 Mile Smiles offers several flexible payment options to our patients. In addition to accepting cash, check, and credit card, we also accept CareCredit.
Financing your balance through CareCredit provides you with easy terms and zero percent financing, so that you get the treatment you need without breaking the bank. Additionally, we can hold your treatment plan until you have saved or arranged financing. We have had patients with fixed incomes save for a few years in order to get the implants that changed their life! Don’t let finances stand in the way of the dental care you need.
